Survey Completion Guide

Everything you need to know about completing your 360° feedback survey

Welcome!

You've been asked to provide feedback as part of a 360° assessment. Your input is incredibly valuable — it helps your colleague understand their strengths and identify areas for development.

Your responses are confidential. Individual ratings are never shared directly with the person being assessed. Results are aggregated by category (e.g., "Peers" or "Direct Reports") so your identity is protected.

Completing the Survey

Step-by-step guide to answering questions

Starting the Survey

When you open a survey, you'll first see an Introduction Page that tells you:

  • Who you are providing feedback for
  • What rater category you're in (e.g. Peer, Manager)
  • Brief instructions on how to complete the survey

Click Start Survey when you're ready to begin.

Answering Questions

Each question asks you to rate an aspect of the person's behaviour or performance. Here's how the rating scale works:

1
Strongly Disagree
This behaviour is rarely or never observed. Significant improvement needed.
2
Disagree
This behaviour is occasionally observed but not consistently. Room for improvement.
3
Neutral
This behaviour is demonstrated to an adequate standard. Meets expectations.
4
Agree
This behaviour is frequently demonstrated to a good standard. Above expectations.
5
Strongly Agree
This behaviour is consistently demonstrated at an exceptional level.
"Not Observed" Option

If you haven't had the opportunity to observe the behaviour described in, select Not Observed. This is perfectly fine — it's better to mark "Not Observed" than to guess.

Only use "Not Observed" when you genuinely haven't seen the behaviour. Don't use it to avoid giving a low score.

Adding Comments

Each question has an optional comment field where you can provide additional context. Comments are especially useful for:

Positive Examples

Share a specific instance when the behaviour was demonstrated well.

Suggestions

Offer constructive ideas for how the person could improve.

Context

Explain any circumstances that influenced your rating.

Tip: Specific examples are far more helpful than vague statements. Instead of "Good communicator", try "Clearly explained the project timeline in the Q3 planning meeting."

Navigating Between Pages

Questions are presented in pages (typically 5 questions per page). Use the navigation buttons to move through:

Previous Page
Next Page
  • Your answers are saved automatically as you move between pages

  1. Click the same link from your invitation email, or go to My Surveys.
  2. Find the survey (it will show as In Progress).
  3. Click Continue and you'll pick up right where you left off.

Tips for Great Feedback

How to make your feedback as helpful as possible

Do
  • Be honest and constructive in your ratings
  • Base your ratings on what you've observed
  • Provide specific examples in comments
  • Consider the person's role and responsibilities
  • Use the full range of the scale (1–5)
  • Take your time — thoughtful feedback is better
  • Use "Not Observed" when appropriate
Don't
  • Give all 5s or all 1s without genuine thought
  • Let a single incident colour all your ratings
  • Include personally identifying information in comments
  • Rush through just to get it done
  • Rate based on hearsay — only rate what you've seen
  • Compare the person to others — rate them on their own merits

Thank You!

Your feedback makes a real difference. It helps people grow professionally and contributes to a better working environment for everyone.

Frequently Asked Questions

Common questions about the survey process

Yes. Your individual responses are never shown directly to the person being assessed. Results are aggregated by rater category (e.g., all Peer ratings are averaged together). If there are fewer than 2 raters in a category, the responses may be grouped with another category to maintain anonymity.

Most surveys take between 10–20 minutes to complete, depending on the number of questions and how detailed your comments are. Remember, you can save your progress and return later.

No. Once you click Complete Survey, your responses are final. Please make sure you're satisfied with your answers before submitting. You can change answers at any time before final submission, and your progress is saved automatically.

Yes, this is completely normal. You may be assigned surveys for several different people if you work closely with multiple interviewees. Each survey is separate and your responses for each person are independent.

Select "Not Observed" when you haven't had the opportunity to see the behaviour described in the question. This is treated differently from a low score — it simply indicates you don't have enough information to rate that particular area.

Yes! The survey is fully responsive and works on mobile devices, tablets, and desktop computers. However, for the most comfortable experience, we recommend using a tablet or desktop.
